To register for services with Disability Services, students must schedule a consultation with the Coordinator of Disability Services, when an “Application for Services” will be completed. For all disabling conditions for which the student might request accommodations, the student must provide appropriate documentation completed by a qualified professional which includes the information identified below in subparts (1) through (5) as appropriate:
- Students requesting accommodations on the basis of mobility, chronic or disease-related disabilities must provide documentation which:
- identifies the disabling condition(s)
- identifies the functionally inhibitive manifestations of the condition(s) for which accommodations are being requested.
- Students requesting accommodations on the basis of a hearing impairment or deafness must provide documentation consisting of:
- an audiological evaluation and/or audiogram
- an interpretation of the functional implications of the diagnostic data and hearing aid evaluation, when appropriate.
- Students requesting accommodations on the basis of a visual impairment or blindness must provide documentation consisting of:
- an ocular assessment or evaluation from an ophthalmologist
- a low-vision evaluation of residual visual function, when appropriate.
- Students requesting accommodations on the basis of a psychiatric disability, attention deficit disorder or traumatic brain injury must provide the following current documentation as warranted:
- Psychological diagnosis as per Diagnostic and Statistical Manual of Mental Disorders (DSM)
- Prescribed medication, dosages and schedules
- The relative stability of the condition or medication
- Functional impairments attributable to the diagnosis and/or medications for which accommodations are being requested
- Students requesting accommodation on the basis of learning disability must provide documentation of diagnostic testing suitable to the adult population by a qualified professional which adequately verifies the nature and extent of the disability in accordance with current professional standards and techniques. To receive services on the basis of a learning disability, students must meet the criteria for diagnosis of specific learning disability.
